Indiana Health Information Exchange, Inc. (“IHIE” or “we”), is committed to protecting your privacy. This Privacy Policy sets forth how IHIE collects, uses and discloses the information we receive through your uses and navigation of www.ihie.org (the “Website”). This Privacy Policy may change from time to time. Please refer to this page periodically to check for updates. Please refer to your subscription or other applicable agreement with IHIE for terms and conditions governing IHIE’s products and services

Scope of the Policy

This Privacy Policy applies to the Website and any subdomain of the Website, unless otherwise explicitly stated, whether accessed by way of a web browser, mobile device or other method.

Information Collected by IHIE

IHIE may collect certain types of information from you when you use or navigate the Website.

Personal Information. IHIE may collect personally identifiable information, such as your name and email address (together, “Personal Information”), from you when such Personal Information is provided to IHIE voluntarily. Please note that Personal Information DOES NOT include your health information. IHIE does not receive or provide health information through the Website. If you have questions regarding your health information, please contact your health care provider.

Usage Information. IHIE may collect additional information from you when you use and navigate the Website. Such information may not identify you as the source and may include information regarding your computer, IP address, Internet browser, location, operating system, areas of the Website visited, actions performed on the Website, and the domain name of the website from which you linked to the Website, and other similar information (together, “Usage Information”).

Method of Collection

IHIE may collect Personal Information from you when you voluntarily provide it through form submission, contact requests, subscriptions to news alerts or other information, event registration, and similar activities. IHIE may collect Usage Information by automatically recording information transmitted by your Internet browser and by using automated tracking technology for Website access and interaction. IHIE may use cookies, which are small data files that are transferred to an Internet browser, to allow the Website to remember your subsequent visits. Please note that opting out from communications or modifying, updating, or deleting your Personal Information will not result in the modification, updating, or deletion of any Personal Information that may reside in back-up or disaster-recovery storage.

Security
IHIE takes reasonable measures to protect your information; however, data transmitted over or accessible through the internet is not guaranteed to be secure. IHIE does not warrant or represent that your information will be completely secure from misappropriation by bad actors or in the event of a network, hardware or software failure. In the event IHIE becomes aware of a security breach involving your information, that is stored by or for IHIE, we will comply with applicable state and federal law and provide notice to you as needed. IHIE is not responsible for disclosures due to the unauthorized acts of third parties or errors in transmission. Please do not send sensitive or confidential information to IHIE through the Website or through email unless you receive authorization from IHIE to do so.

GDPR

Please note that IHIE does not maintain any type of establishment in the European Union and does not direct or target individuals located in the European Union. IHIE is not directly subject to the GDPR and maintains its data processing activities in the United States, which may not offer the same protection as GDPR.

Do Not Track

The Website does not respond to Do Not Track signals that your web browser or device may use to indicate your preference not to be tracked online.
