Public Reporting

The quality measure reports published on this site detail the quarterly performance of physician practices participating in IHIE’s Quality Health First® Program (QHF®).

Practice sites included in the report have agreed to the public reporting of their scores and must have a minimum of 30 patients in any measure. Practices may be comprised of several physicians, including internists, family practitioners, and pediatricians, as well as nurse practitioners, and physician assistants.

Clinical quality information for all providers participating in the QHF® Program is included in the community measure scores.

The QHF Clinical Quality Measures scores are publicly reported twice each year. The current reports consist of quarterly scores as of 9/30/2012. Clicking on the desired region below will present reports showing the quality scores for all practice sites by measure and reports with measures by practice site. (Regional geography is represented in the map to the left). About the QHF® Program

The Indiana Health Information Exchange is committed to reporting the impact that the Quality Health First® (QHF) Program has on the community. The QHF Program is unlike any other similar public reporting effort in the country for several reasons:

• The QHF Program provides a truly comprehensive community report. It includes over one million patients.
• Physician practices are able to review and verify the data in the quality measure performance reports provided to them. The physician is able to update the data on the basis of information contained in the patient medical records on file.
• The QHF Program provides physicians and patients with comprehensive health data to help improve the health of the community by monitoring and managing chronic diseases.
• The QHF Program reflects the collaborative efforts of Provider Groups, Payors, and Employers in the State of Indiana.

About the Results

The goal of the Quality Health First® Program is to improve patient care by giving providers tools and information to monitor patients' health and wellness and better manage chronic diseases. The results are based upon available patient data provided by Quality Health First® Program participants and collaborating organizations. The results reflect the percentage of patients that received the care called for by the measure during the measurement period.
