The quality measure reports published on this site detail the quarterly performance of physician practices participating in IHIE’s Quality Health First® Program (QHF®).
Practice sites included in the report have agreed to the public reporting of their scores and must have a minimum of 30 patients in any measure. Practices may be comprised of several physicians, including internists, family practitioners, and pediatricians, as well as nurse practitioners, and physician assistants.
Clinical quality information for all providers participating in the QHF® Program is included in the community measure scores.

Clicking on the desired region below will present reports showing the quality scores for all practice sites by measure and reports with measures by practice site. About the Results
The goal of the Quality Health First® Program is to improve patient care by giving providers tools and information to monitor patients' health and wellness and better manage chronic diseases. The results are based upon available patient data provided by Quality Health First® Program participants and collaborating organizations. The results reflect the percentage of patients that received the care called for by the measure during the measurement period.
