Heart Health, Respiratory and Other Measures

Descriptions of Women's Health Measures
Breast Cancer Screening: Percentage of women 40–69 years of age who had a mammogram to screen for breast cancer during the previous 24 months.
Cervical Cancer Screening: Percentage of women 21–64 years of age who received one or more Pap tests to screen for cervical cancer during the previous 36 months.
Chlamydia Screening: Percentage of women 16-24 years of age identified as sexually active, who had at least one test for chlamydia during the previous 12 months.

Descriptions of Heart Health, Respiratory and Other Measures
LDL-C Screening for Patients with Cardiovascular Conditions: Percentage of patients 18–75 years of age with certain coronary artery disease diagnoses who received an LDL-C screening test during the previous 12 months.
LDL-C Control at <100 mg/dL for Patients with Cardiovascular Conditions: Percentage of patients 18–75 years of age with certain coronary artery disease diagnoses who have their LDL-C level controlled at <100 mg/dl during the previous 12 months.
Lipid Profile for Patients with Coronary Artery Disease (CAD): Percentage of patients 18 years of age and older with a diagnosis of CAD who received at least one lipid profile (total cholesterol, HDL-C, LDL-C, triglycerides) during the previous 12 months.
Use of Appropriate Medications for Patients with Asthma: Percentage of patients 5–64 years of age diagnosed with persistent asthma who were prescribed appropriate medication during the previous 12 months.
Colorectal Cancer Screening: Percentage of patients 50–75 years of age who had an appropriate screening test done for colorectal cancer.
Use of Imaging Studies for Low Back Pain: Percentage of patients 18–50 years of age with a primary diagnosis of low back pain during the previous 12 months who did NOT have an imaging study (plain X-ray, MRI, CT scan) within 28 days of the diagnosis.

Descriptions of Diabetes Measures
HbA1c Testing for Patients with Diabetes: Percentage of patients 18–75 years of age with type 1 or type 2 diabetes who had HbA1c testing during the previous 12 months.
HbA1c Control ≤9% for Patients with Diabetes: Percentage of patients 18–75 years of age with type 1 or type 2 diabetes with HbA1c level controlled (≤9.0%) on their most recent HbA1c testing during the previous 12 months.
HbA1c Good Control <8% for Patients with Diabetes: Percentage of patients 18–75 years of age with type 1 or type 2 diabetes with good HbA1c control (<8.0%) on their most recent HbA1c test during the previous 12 months.
LDL-C Screening for Patients with Diabetes: Percentage of patients 18–75 years of age with type 1 or type 2 diabetes who had an LDL-C screening test done during the previous 12 months.
LDL-C Controlled at <100 mg/dL for Patients with Diabetes: Percentage of patients 18–75 years of age with type 1 or type 2 diabetes whose most recent LDL-C level in the previous 12 months was controlled at <100 mg/dL.
Kidney Disease Monitored for Patients with Diabetes: Percentage of patients 18–75 years of age with type 1 or type 2 diabetes who were monitored for kidney disease during the previous 12 months.
Retinal Eye Exam for Patients with Diabetes: Percentage of patients 18–75 years of age with type 1 or type 2 diabetes who had a retinal eye exam during the previous 12 months or a negative retinal eye exam during the previous 24 months.

Descriptions of Children's Health Measures
Well-Child Visits in the First 15 Months of Life: Percentage of children who had six or more well-child visits during their first 15 months of life.
Well-Child Visits for Children Three-to-Six Years of Age: Percentage of children 3–6 years of age who received one or more well-child visits with a primary care practitioner each year of age.
Adolescent Well-Care Visits: Percentage of adolescents 12-21 years of age who received one or more well-care visits with a primary care practitioner each year of age.
Appropriate Testing for Children with Pharyngitis: Percentage of children 2-18 years of age diagnosed with pharyngitis and prescribed an antibiotic who were tested for Group A Streptococcus.
Appropriate Treatment for Children with Upper Respiratory Infection: Percentage of children 3 months–18 years of age who were diagnosed with upper respiratory infection (URI) not requiring an antibiotic, and were not prescribed an antibiotic.
Previous Public Reports
The goal of the Quality Health First® Program is to improve patient care by giving providers tools and information to monitor patients' health and wellness and better manage chronic diseases. The results are based upon available patient data provided by Quality Health First® Program participants and collaborating organizations. The results reflect the percentage of patients that received the care called for by the measure during the measurement period.
Please note Most Recent Quarter Reported is 3/31/12, and each of the Measures also show First Quarter Reported which varies depending on when the measure was initiated.
