At Valley Professionals Community Health Center (VPCHC), the Care Coordinators complete post-hospital or ER visit reports and ensure documents are uploaded for the provider to view. In order to do this, Care Coordinators need access to health records from facilities outside their primary EHR system—Cerner.


When a patient is discharged, Care Coordinators utilize CareWeb to print documents and then complete timely follow up with patients to ensure gaps in care are closed. These records also help providers view what testing was already done in order to further treat patients and avoid redundancy.
