There was a patient admitted to Ascension St. Vincent Hospital for
Neurosurgery. A Care Coordinator was assigned and needed to follow the patient’s status to update the primary care physician (PCP) with significant changes and the status of the patient’s outcome. Unfortunately, the Care Coordinator did not have access to Ascension St. Vincent Hospital charting system.


IHIE’s product, CareWeb, allows Care Coordinators to monitor updates and progress notes on patients’ statuses to update PCPs on progress. The tool also enables the PCP to obtain records quickly instead of sifting through traditional electronic medical records allowing the physician to spend more time with the patient.
