Problem
CICOA Aging & In-Home Solutions connects older adults, people with disabilities and family caregivers in central Indiana with home and community-based services. CICOA is working with Medicaid on a project to support Medicaid patients that use CICOA for additional services. CICOA's clients often have multiple conditions that require care, so this project helps CICOA manage these clients, with the ultimate goal to help people age in place and reduce unnecessary institutionalizations.
Solution
CICOA began using CareWeb to support this project. With CareWeb, CICOA care managers are able to see the care that their clients have received, which enables them to conduct effective one-on-one follow-ups. For example, CICOA uses this information to help ensure their clients are taking their medicine and taking it at the proper dosage. It also enables them to be a voice for the patient, especially if the patient has forgotten to tell doctors important health information which could impact treatment.

CICOA hopes to expand their use of CareWeb to support their entire client base and help bridge gaps – from clinical care, to transportation, to home modifications support – for their community’s most vulnerable populations.
