Quality Health First Program

Diabetes. Heart disease. Breast Cancer. Asthma.

In over 70 communities in Indiana, over 1,900 physicians are using the Quality Health First Program to help them identify, prevent and manage these conditions, and other conditions in their patients. Patients needing interventions are flagged for their providers, allowing them to make the best care decisions possible.

The program utilizes information from the Indiana Network for Patient Care to assemble lab results, immunizations, diagnoses, cardiology and radiology reports, procedures and medication information. This information is then organized in easy-to-read patient summaries that can be used before, during or after patient visits to make the best use of physicians' time.

Impact on Patient Care:

  • Allows physicians to track, in real-time, those patients who are due for preventive screenings.
  • Provides reminders for health screening interventions to make the best use of patient appointments.
  • Enables early interventions and consistent support to those patients with chronic diseases.
  • Includes disease-based summaries based on the physician’s patient population.
  • Provides physicians with comparisons of their care to the physician community as whole as a way for the program to help identify best-practices.
  • Supports two-way information flow, enabling physician offices to update information regularly to ensure accuracy of program reporting.

Impact on Healthcare Reform:

  • Involves payers so that reimbursement to physicians from payers aligns with improvements in patient outcomes.
  • Helps providers meet certain Meaningful Use criteria outlined in the Health Information Technology for Economic and Clinical Health (HITECH) Act by linking the use of health information technology to achieving measurable outcomes in patient engagement, care coordination and population health.

The program includes participation from all major healthcare stakeholders, and provides value to patients, hospitals, physicians, payers, employers, public health and others.
